Student Health Services offers psychiatry services through a coordinated care model.

The medical care team provides team-based psychiatric care for 91°µÍø students in a timely and supportive way.

Prior to Your First Appointment

Before your initial psychiatry visit to Student Health Services, you must submit the following information (fax or mail):

• Initial diagnosis from a license provider, or nurse practitioner (NP) -OR- a referral from Student Counseling Service. Note, there is not a form to complete. 
• Medical and medication history (past three months).
