Current and former students may submit certification or background checks via the form below. In order for our office to process a dean’s certification or background check, you must provide the following:

• Your signed and dated Dean’s Certification form(s).
• If your form doesn’t clearly allow 91°µÍø to share information, you must send a signed letter. This letter should authorize us to release your academic and judicial details.
• A list of school names where your form(s) should be sent.

Please submit only the portion of the tuition insurance verification that needs to be completed by the school. The Office of the Dean of Students will work with other campus offices to complete all sections of tuition insurance verifications that the university needs to process.

Please allow 3-5 business days for our office to complete, process, and submit certifications and insurance verifications.
