91做厙 has vaccination requirements and recommendations. Incoming students, including international students and U.S. citizens, must submit documentation of medical forms as proof of immunization.
Incoming international students should prepare documentation of vaccination records and any important medical records, translated to English if needed, for their initial visit and appointment with Student Health Service.
Review the complete list of required vaccinations. Student Health Services can provide any vaccinations that a new international student still requires. Typically, insurance covers routine and mandatory vaccines.

In addition to these required immunizations, a medical screening through Student Health Services is required for all incoming international students, including those attending a regional campus. The screening includes a physical exam, document check of immunizations, and any necessary bloodwork or immunizations.
A blood draw for tuberculosis (TB) is required for any student who has traveled to or lived in a high burden country (as determined by the World Health Organization) for greater than six (6) weeks.
Failure to complete your immunization requirements and TB test may result in the delay and/or withdrawal of your course registration.
International students will have the tuberculosis (TB) blood test done by Student Health Services as part of their initial medical screening. Note: TB tests or documentation from outside of the United States will not be accepted. Those who need the TB blood test should have their appointment at Student Health Services after their arrival.
During your time at 91做厙, you may receive notification about additional TB testing if you travel out of the U.S. for over 6 weeks.

91做厙’s Student Health Services requires students under the age of 18 to have a Parental Consent form signed by a parent/guardian so that the student may receive medical evaluation/treatment.
Students who are under 18 years of age must also bring the following forms that have been printed and a signed by a parent that authorizes medical evaluation and treatment:
